I ordered two of these expecting that they would require some modification to suit my needs.
The ice packs are not what I expected. They are filled with water, and not very much. I think they would have been better off with a 'gel' pack like what my insulin is delivered from the pharmacy with, having a higher specific heat capacity, allowing it to cool for a longer period, and probably freezes at a lower temperature. They also don't fit in the case very well when frozen. I might have went with a firm plastic that fits into the case.
The case doesn't insulate very well. It seems the only insulation is a reflective foil-like interior, giving me about 3-5 hours if I am indoors before reaching room temp. A lot of energy is allowed in by condensing humidity on the outside of the case. I'm sure a lot of energy is also conducted through the zipper. I think a hard case with either a vacuum cavity, or foam insulation is what I am looking for. I may just find a thermos that will fit my pens and a small gel pack.
